I drive a Mercedes C220 CDI. Year 2003. It' an automatic. As you will no doubt be aware - there is an option to select C or S at the left side of gear stick.
Firstly I believe these mean Comfort or Standard. Is this correct?
Secondly does anyone genuinely know which option will provide the most MPG?

Hi and welcome.
Yes it does mean sport and comfort. In general Comfort should give marginally better economy as it changes up earlier compared to Sport.

Can't remember from my S203, but with the S204 I leave it in C most of the time - except approaching roundabouts and motorway slip roads as there is a noticable hesitation from stationary when pulling onto a roundabout, nearly had me T-boned a couple of times when in C. Not as bad as in S, but it's a foible you have to get used to!!

C starts in 2nd gear , S starts in 1st gear and Holds the revs higher before changing up .
