If the rot isn't too serious, ie just the bottom of the cross member has gone (which is usually the case) I would get it cut away and a new bottom welded in, I'm sure I've read somewhere that a repair section is available, if not its just a flat plate that could easily be fabricated. To replace the cross member completely is a horrendous job that involves supporting the engine and chassis legs with a special jig.

Was an E300TD, now a Lexus LS400
Is it surface rust, or rusting through?

As mentioned, they can be repaired in-situ.

If the cross member is that bad then I would have a serious look at the front spring upper mounts (perches), chances are they will be rotten too, the consequences if they fail can be dramatic!
