Poor running when cold.

On cold days my C220cdi auto runs ok until it changes into top gear, then for the next few miles it wont run at steady revs. The revs cycle over about a 200 rev range with the engine note rising and falling. This stops after a few miles but is very unnerving whilst it is happening. Does anybody know what is happening and what remedy is? Any advice much appreciated.
