W210/E320 Avantgarde engine rough idle.

My year 2000 W210/E320 Avantgarde recently developed a rough idle. Can sense it with body shake and exhaust note. No problem under load and normal smooth acceleration and cruise. Engine mounts replaced 50,000 k's back and engineer inspection reports mounts to be still OK.

BITE shows no malfunctions. Total Km's driven 156,000

Instinct tells me that if an ignition fault it would still be there during normal drive.

Where should I look next ?

Alternator pulley? - the clutches fail within them.

Injector tip wear, mine did the same at 60k miles, pulled the injectors and 3 were badly worn affecting the spray pattern, an injector refurb sorted it. Symptoms exactly as you describe fault was only evident on tickover as soon as revs were increased normal running

Take the car to a local diesel specialist they will remove the injectors and bench test

Just reread your post if the car is petrol ignore the above

I suspect it's petrol since he talks about ignition issue in which case no alternator pulley clutch.
I suspect it's a manifold leak. Very common for manifold leaks to only affect low speed running.

With the correct tool you can do a smooth running test on the engine and see which cylinders are misfiring. Usually one or two HT leads are the cause.
