Won't change up into top till 4500rpm. Have a look at the side of the air filter. You will see the throtle linkage. At the bottom there is a cable attched with a small white\cream plastic piece. On it you will see a knurled plastic nut with an arrow like marker. Adust it till the marker is in the middle of the gap. You will see what I am talking about. Or try removing the clip all together. A sharp blow with a screw driver will remove the clip. Run the vehicle if this cures it then the cable requires adjustment. Ie adust till the marker is in the middle. This cable is called the "control pressure cable".
As for the thump. If the thump is only in one gear change then its more than likely a problem internal in the box. If all the changes are harsh. Check that the vacuum pipes are on the side of the box and are connectied engine side.
