C180 sounds a bit like a bus.

I have a W203 C180 petrol and when reversing at very low revs, it tends to sound a bit like a bus. The same rough noise can be heard for a split second when I start the engine. It is not a nice sound but doesnt effect the performance of the car. Can anyone shed some light?

Check your engine mounts.

If it is the engine mounts, would it be vibration that is making the noise? If so, what would a fix involve, is it a tighten up or something more costly?

Is the engine the 111 version? If so can you repeat the noise if you rev the engine gently to about 1600 then let go of the throttle ? If you can then its the cam adjuster a common fault and nothing to worry about as repares are very expensive.
