But is in fact owner stupidity?

A. Not fully tightening the wheel nuts.... :(

Just completed B service. Used a new wheel wrench, big long bar job, didn't want to over-tighten (in case I couldn't get undone with standard wrench away from home). Too lazy to use torque wrench.

150 miles all the way to work and back saying to myself, "That noise is really like hub wear, can't possisbly be, I checked the play for the service???".

Oops.

I once had a replacement car from the company garage, whilst mine was being serviced there. 'It chattered' on fast bends and that was loose front wheel nuts.
Next day, when I gave the garage foreman a rocket he said "we can't check everything you know"!!!
You can tell we always had to be ordered to use the company garage and not an outside one can't you?
