Can't get the year but do have the build sheet.
Yours is a 722.409 gearbox. You need the exact one specified per the build sheet (as shown above).

One thing to check - how is the ATF level?

Also before changing the gearbox remove the pan from the box and check for metal in the oil and filter. It may be something very simple.

Thanks everyone. I deleted the original question but here it is again: 1990 (I think) 190e 2.6 LHD, with no forward drive but reverse works fine. I need to know which auto boxes from which Mercedes cars will fit please? Car has done about 200k but has always been well maintained and not driven hard. Thank you.

Only the 190E 2.6 or 260E (<1993) used this gearbox. It must be a 722.409 (W 4 A 020) to guarantee correct fit. The numbers will be stamped/cast on the casing. Have it looked at first. It may be something simple.
