Tappets - an old nail

My old nail of a 280TE sounds like a bag of spanners at any speed. It appears to be No 4 exhaust tappet, but having checked the clearance, it is within specification, probably tight if anything. @ 0.25mm/10 thou

Should I narrow the clearance on this exhaust valve further - at a risk of valve seal burning etc, or open it out to reduce the noise?

If it's noisy and the tappet is set OK, then the cam lobe is probably worn.

Spanners- short of replacing the cam shaft, is there anything I can do about it?

Normally when the cam lobes are worn out, you cannot reduce the tappet clearance to the correct setting. Assuming all the other tappets are correctly set, is oil reaching the tappets? My partners Alfa sounds like a diesel for about a second on startup until the oil gets through!
