W124 280E petrol 1993 cold start problems

My 280E won’t start from cold anymore unless it gets a small bit of accelerator pressure, and when it does start it sounds as if the fan belt is squealing. The engine then hunts between 500 and 1000 revs for about 2 mins. Once the load goes on the car the squealing stops and does not come back until the car is started from cold again, and the idle problem also disappears until the next time the car is started cold. It seems all these problems are related in some way as they all disappear once the engine is warm.

The squeal is more than likely the fan belt needs adjustment or replacing
